The short version
Remineralizing distilled or reverse osmosis water is a taste and equipment decision, not a health rescue. The widely repeated figures that low mineral water is dangerous, and that you need at least 10 mg/L of magnesium and 20 mg/L of calcium, come from a 2005 review chapter that the World Health Organization’s own current guideline has since superseded. WHO now proposes no minimum at all. That does not make remineralizing pointless: it fixes flat taste, brings the pH back toward neutral, and is genuinely required for aquariums, coffee, and hydroponics. The most consistent method is an inline cartridge after the membrane. The cheapest is a few drops. Neither is doing anything for your health that dinner is not already doing better.

Why Distilled Water Tastes Flat
Distillation and reverse osmosis are both non-selective. Neither one can tell the difference between lead and calcium, so the same process that strips out the contaminants strips out the dissolved minerals that give water its taste. What is left is chemically clean and, to most palates, empty. It also tends to sit slightly on the acidic side of neutral, because with the mineral buffering gone, dissolved carbon dioxide from the air has more effect on the pH than it would in ordinary tap water.
Remineralizing means putting some of that back after filtration. Calcium and magnesium are the two that matter for taste, and bicarbonate is what restores the buffering. Do it and the water tastes rounder and reads closer to neutral on a pH strip. That is the entire mechanism, and it is worth being clear about it, because a large amount of what gets written about remineralization attaches a health story to a taste change.

The Health Claim, and Where the Numbers Came From
Almost every article on this subject cites the same figures: a minimum of 10 mg/L magnesium and 20 mg/L calcium, sometimes with a claim that drinking demineralized water leaches minerals out of your body. Those numbers are real, they have a traceable origin, and that origin is not what most people think it is.
They come from a chapter by Frantisek Kozisek titled Health Risks from Drinking Demineralised Water, published in the WHO volume Nutrients in Drinking Water in 2005. It was a literature review, drawing largely on older Russian and Czech animal and human studies, and it was never a WHO guideline value.
What happened next is the part that gets left out. WHO revisited the question at an expert meeting in 2006, published a background document on hardness in 2011, and folded the conclusion into the current Guidelines for Drinking-water Quality. That guideline states that while there is epidemiological evidence for a protective effect of magnesium or hardness on cardiovascular mortality, the evidence is debated and does not prove causality, that there are insufficient data to suggest either minimum or maximum mineral concentrations, and that therefore no guideline values are proposed.
Germany’s Federal Institute for Risk Assessment went through the same literature in 2020 and put it plainly: the statements in the 2005 chapter are outdated, and the later WHO documents no longer cite those studies. Its own conclusion was that for people eating a balanced diet and drinking normal quantities, no adverse effects can be identified that would argue against drinking very low mineral water.
That assessment did name specific groups who should pay attention to their total daily calcium and magnesium if very low mineral water is their main source of fluid, and those exceptions are worth keeping rather than waving away:
- People on very unbalanced diets, or who deliberately avoid whole food groups, including anyone avoiding dairy for lactose intolerance or on a vegan diet.
- Pregnant and breastfeeding women, whose mineral requirements are higher.
- Anyone exercising hard for longer than about 90 minutes, where very low mineral water is specifically not recommended as the main drink for fluid replacement, mostly because it supplies no sodium and no carbohydrate.
The reason the general case is undramatic is arithmetic. In the German national dietary survey the institute drew on, bottled water accounted for roughly 8 percent of median daily magnesium intake. The other 92 percent came from food. Adding minerals to your drinking water moves a small number, and it moves it whether the minerals came from a cartridge or from a slice of bread.
What this means in practice. Remineralize because you want the water to taste better, because your aquarium or espresso machine needs it, or because you prefer a neutral pH. Those are good reasons and they are easy to satisfy. Do not do it because you have been told RO water will pull minerals out of your bones, and be wary of any product page that says so, because that claim rests on a document its own publisher has moved past.
The Methods, Compared
Five approaches cover essentially everything sold. They differ mainly in how consistent the result is and how much you have to think about it:

The inline cartridge is the default answer for a plumbed system because it acts on every glass without a decision. It sits as the last stage, after the membrane and after the storage tank if there is one, and gets replaced every six to twelve months like the other cartridges. The media is not exotic: calcite is crushed calcium carbonate, essentially limestone or marble, and it dissolves slowly to add calcium and raise pH. Magnesium oxide, sold under the trade name Corosex, adds magnesium and pushes pH harder, which is why it is normally blended as the minor partner rather than used alone.
The salt method deserves a straight answer, since it is the one most often repeated. Unrefined sea salt and pink Himalayan salt do contain trace calcium and magnesium, but they are overwhelmingly sodium chloride. A pinch large enough to move calcium or magnesium meaningfully is a pinch large enough to make the water taste salty and to add sodium you probably were not planning on. It is a cheap way to change the flavour, not a sound way to remineralize.
How Much You Are Actually Adding
There is no certification standard for remineralization performance. NSF/ANSI 58 governs what a reverse osmosis system takes out, and nothing in it verifies what a post-filter puts back, so cartridge claims are manufacturer claims. That makes a cheap TDS meter the only practical check you have.
Measure at three points: your tap water, the RO output before the mineral stage if you can reach it, and the finished water. A working cartridge shows up as a clear rise between the second and third readings. Owners who do not know the stage is there often mistake that reading for a failing membrane.
Two limits on what the meter tells you. It infers dissolved solids from electrical conductivity, so it reports that something is dissolved in the water, not which minerals or in what proportion. And a reading that climbs steadily over months usually means the media is being consumed unevenly rather than that the water is improving. If you want to know your actual calcium and magnesium in mg/L, that takes a lab test, and for drinking water it is rarely worth the money.
Where Remineralizing Genuinely Matters
Set the health argument aside and there are still cases where pure water is the wrong input and remineralizing is not optional:
- Freshwater and reef aquariums. RO or RO/DI water is the standard starting point precisely because it is a blank slate, and it has to be brought up to a target general hardness and carbonate hardness with a purpose-made salt mix before it goes anywhere near livestock. Straight distilled water in a tank is not gentle, it is lethal.
- Coffee and espresso. Extraction depends on minerals in the water, and completely demineralized water pulls a thin, sour shot. It is also a poor idea in machines that sense water level by conductivity, where near-zero water can read as an empty tank.
- Hydroponics. Growers start from RO water for the same reason aquarists do, then build the nutrient profile deliberately rather than fighting whatever the tap supplies.
- Cooking, at the margins. Very low mineral water leaches more calcium and magnesium out of vegetables during boiling. It is a real effect and a small one against a normal diet.
The common thread is that none of these are about you. They are about a downstream process that was designed around water containing something.
Never add mineral drops or salt to water for infant formula. Formula is a complete food, formulated to deliver the right mineral load once mixed to the manufacturer’s instructions, and adding minerals on top of it changes the solute load an infant’s kidneys have to handle. Low mineral water is not the problem here: risk assessors have specifically found that preparing formula with very low mineral water does not produce inadequate mineral intake. This Is Not the Same as Alkaline Water
Calcite and magnesium oxide raise pH as a side effect of dissolving, which is why remineralization cartridges are frequently marketed as alkaline filters. The two words end up describing the same hardware, and the marketing quietly shifts from a taste claim to a health claim along the way.
Worth separating. Bringing slightly acidic RO water back toward neutral is a real and modest change, and it is a reasonable thing to want. The broader claims made for alkaline water, that a higher drinking pH changes your body’s pH or delivers specific health benefits, are a different proposition and are not well supported. Your stomach maintains a strongly acidic environment regardless of what you drink. Buy the cartridge for the minerals and the taste, and treat the pH number on the box as a side effect rather than the point.

Frequently Asked Questions
Is distilled or RO water bad for you if you do not remineralize it?
Not for most people. The current WHO drinking water guideline proposes no minimum mineral concentration, on the grounds that the data are insufficient to set one, and a 2020 German risk assessment found no adverse effects for people on a balanced diet drinking normal quantities. The groups told to watch their totals are those on very restricted diets, pregnant and breastfeeding women, and anyone exercising hard for over 90 minutes.
How much salt should I add to distilled water?
None, if the goal is minerals. Sea salt and pink Himalayan salt are overwhelmingly sodium chloride with only traces of calcium and magnesium, so any amount that moves those numbers also makes the water taste salty and adds sodium. Mineral drops or a calcite cartridge do the job that salt is being asked to do here.
Does a remineralization filter reduce the purity of RO water?
No. It sits after the membrane and adds specific minerals to water that has already been filtered. It does not put contaminants back and it does not affect what the membrane removed. What it does change is your TDS reading, which is why finished water from a system with an alkaline stage measures higher than water taken straight off the membrane.
What TDS should remineralized water read?
There is no standard to hold it to, since no certification covers what a post-filter adds. In practice, systems with a mineral stage commonly finish somewhere in the region of 50 to 80 ppm against near-zero off the membrane. The useful test is the change between before and after, not the absolute number.
Can I just mix distilled water with tap water?
You can, and for an aquarium where you are targeting a hardness figure it is a normal technique. For drinking water it defeats the purpose, since whatever you filtered out goes back in proportionally. If you removed lead or PFAS, blending returns a share of them along with the calcium.
Do mineral drops make water alkaline?
They raise the pH somewhat, as does a calcite cartridge, because the minerals dissolving into the water buffer it back toward neutral. That is a side effect of remineralizing rather than a separate benefit, and the broader health claims made for alkaline drinking water are not well supported.
